On examination, he was found to have hoarseness, vocal cord paralysis, and a 6 cm left neck mass. He has no prior history of malignancy. Imaging shows the mass involving the left thyroid lobe, displacing the superior trachea and esophagus, and extending into the superior mediastinum. Laboratory evaluation, including serum calcium, thyroid function tests, and parathyroid hormone level, was within normal limits. Imaging, a fine-needle aspirate, and core needle biopsy are shown below, followed by a composite resection specimen.
