Training > Clinical Fellowships > Selective (Surgical) Pathology

The fellowship is divided into seven rotations, each approximately seven weeks in length.

Merit is central to the selection process of fellowship canidates since fellows are given major clinical responsibilities with appropriate staff supervision. Preference will be shown to our own residents when an inside and outside candidate are judged to be equal in diagnostic skills, judgment, and maturity.

Rotations

Gross Room

This fellow assumes responsibility for organizing all work in the gross surgical pathology laboratory and distributes cases to residents and medical students. He/she takes 25 percent or a proportional share of all daily cases for personal prosection and independent signout at a junior staff level. Also, the fellow is responsible for teaching other house staff and students in gross prosection and for supervising their work. The fellow resolves all problems regarding accessioning and histologic preparations of surgical pathology cases at Barnes-Jewish Hospital South Campus. The gross room fellow also performs frozen section examinations at St. Louis Children's Hospital.

South Campus Hot Seat

This fellow tracks all surgical pathology and hematopathology cases at Barnes-Jewish Hospital South Campus, making sure that all cases that are viewed with staff are signed out and dictated the same day by other house staff. He/she examines histologic slides of all cases prior to making them available to residents and students. The fellow takes all telephone calls from clinical staff on active surgical/hematopathology cases and communicates results of daily rush biopsies verbally to clinical services.

Frozen Section

These individuals function as the frozen section team and, therefore, perform all frozen sections on inpatients at Barnes-Jewish Hospital South Campus. These fellows are backed up by staff. The fellows on these cases are responsible for the primary diagnosis at frozen section and are only required to call staff when they cannot satisfactorily answer questions. Due to the volume and complexity of case material, the fellows learn to manage and operate in a busy and complex environment. One of the fellows also assists the neuropathology fellow with frozen sections.

Consult

The fellow assigned to consults logs in all consultation cases sent with referral patients to Barnes-Jewish Hospital South Campus ("outside/inside" cases), as well as those sent to individual staff members in surgical pathology. He/she works up all consultation cases prior to presentation and sign out with senior staff; in addition, the fellow takes beeper call on all consultation-related telephone cases and performs frozen section examinations at St. Louis Children's Hospital.

North Campus Hot Seat

The Hot Seat Fellow at Barnes-Jewish Hospital North Campus tracks all active surgical and hematopathology cases at that institution and makes sure they are promptly signed out and dictated by other house staff. He/she examines microscopic slides on all active cases in these disciplines, for communication of provisional diagnoses to clinical staff. The fellow takes all telephone calls relating to surgical or hemato-pathology diagnoses, verbally communicates results of daily rush biopsies to clinicians and advises junior residents in anatomic pathology at Barnes-Jewish Hospital North Campus.

Gynecologic Pathology

The fellow assigned to gynecologic pathology is responsible for supervising and/or diagnosing all cases seen on that service. He/she carries a pager and effectively serves as the GYN pathology "hot seat" during this rotation, as well as overseeing pathology and OB/GYN residents assigned to the service.

Research

All fellows have one rotation in which to pursue research interests or subspecialty disciplines in surgical pathology, as well as to provide coverage for vacations.
